SDAR CP-88-21, "Instrument Tubing Clamps" Gentlemen:
On December 5, 1986, we submitted a Final Report (TXX-6140) on a deficiency involving a lack of detail in project specifications and installation procedures regcrding manufacturers recommendations for torquing of J. C. White tube clamps. As a result of evaluations by our architect / engineer in 1986 and James C. White Co., we concluded this issue did not meet the criteria for reportabilityunder10CFR50.5S(e). As a result of additional review, we have determined this conclusion was reached in error, and we are submitting this letter to provide a revised report.
During tubing support requalification efforts conducted in 1987 by our present architect / engineer, this deficiency resurfaced as an issue. Our reevaluation of this deficiency (docketed SDAR CP-88-21) concluded the deficiencies associated with torquing of these instrument clamps were reportable as noted in our letter (TXX-88140, dated January 29,1988).
