On April 25,1997, the NRC completed an inspection at your Oosd Cities Nuclear Power
Station. ' The purpose of the inspection was to review radiation protection performance
l
during the Unit 2 refueling outage (02R14). The enclosed report presents the results of
l
this inspection.

Overall, good radiological controls were implemented during the outage. In particular, your
i
efforts to include contingent and emergent work dose estimates, based on historical data,
into the dose goals led to conservative outage ALARA planning. This resulted in the
!
implementation of good ALARA controls during high dose jobs such as the use of cameras
i
to monitor undervassel activities and to monitor the replacement of the Unit 2 recirculation
!
pump motor.

We are concerned, however, about two violations of NRC requirements that were
identified. In the f;rst instance, material which had been released from your facility was
i
identified to be contaminated at an offsite location, in the second instance, engineering
controis which were specified for use during work on a Unit 2 reactor water cleanup
i
,
(RWCU) system valve were not used which resulted in a worker's intake of radioactive
'
material. Oversight of and preparation for this job were determined to be ineffective. As
!
stated in the report, the similarity of this event to other events in previous outages
indicates that improvement was needed in the planning and control of valve work.
These violations are cited in the enclosed Notice of Violation (Notice) and the
circumstances surrounding them are described in detail in the enclosed report. Please note

that your are requirnd to respond to this letter and should follow the instructions specified
in the enclosed Notice when preparing your response. The NRC will use your response, in

part, to determine whether further enforcement action is necessary to ensure compliance

with regulatory requirements.
