NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 22-25, 1978, it appeared that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:
A.
Criterion XII of Appendix B to 10 CFR 50 requires that measures shall be established to assure that measuring and testing devices used in activities affecting quality are properly controlled and calibrated at specified per.iods to maintain accuracy within necessary limits.
The QA Manual procedure TT-22-326, Revision 1, pacagraph 2.0, states in part, "Each tool, gage, instrument or other measuring device shall be assigned a serial number, permanently marked....
Each item shall bear a sticker... indicating date of last calibration, calibration due date and initials of the individual responsible for checking."
Paragraph 10.0 states in part, " Certifications shall be furnished to Tube Turns for all calibrations performed by contract sources."
Contrary to the above, the following conditions were observed:
The ammeters on welding machir.es No. 32182 and No. 32048, did a.
not have assigned serial numbers, permanently marked.

For welding machine No. 32108, calibration was not controlled in that the voltmeter and ammeter were marked with serial numbers T-385 and T-386, respectively while on the latest calibration record dated August 9, 1978, the voltmeter and ammeter were identified as serial numbers T-336 and T-385, respectively.

calibration date of April 11, 1978, and a due date of October 11, 1978, whereas, the Instrument Calibration Log Sheet record showed for these instruments, a recalibration date of May 2, 1978, and i

For the 10,000 pound Dead Weight Tester, calibration was not controlled in that the calibration sticker'showed a calibration date of December 1,1977, and a due date of December 1,1978, whereas, the latest calibration record showed a calibration date of April 21, 1978, and a due date of April ' 21,1979.

and No. 41YO6, had not been furnished to Tube Turns.
B.
Criterion XII of Appendix B to 10 CFR 50 and ASME Code NCA-4134.12 require that measures shall be established to assure that measuring devices used in activities affecting quality are properly controlled and calibrated at specified periods to maintain accuracy within nec-essary limits.
Contrary to the above, there were no' measures in effect,for controlling and calibrating the weld shielding gas flow meters on welding machine Nos. 32182, 32048, 32108, and others, nor were these measuring devices controlled and calibrated at specified periods.
C.
Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures.
Procedure TT-17-048, Revision 0, states in part, "All gages and equip-ment under. calibration control must be added to the Instrument Master 4

i Contrary to the above, the Instrument Master File record showed that a unique serial number had not been assigned to the 10,000 pound Dead Weight Tester. and an ammeter, as both devices were assigned serial No.
