Murdock Incorporated Docket No. 99900313/78-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 28-31, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
A.
Criterion V of Appendix B to 10 CFR 50 states in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings...."
Paragraph 9.4.1 of the ASME accepted QA Manual states that, "All pressure gages and measuring equipment will be inventoried and accounted for on an annual basis."
Contrary to the above requirements, an annual inventory and accounting of pressure gages and measuring equipment was never conducted.

Criterion V of Appendix B to 10 CFR 50 states in part that, " Activities

Paragraph 13.7.4 of the ASME accepted QA Manual states that, "The Quality Engineer will prepare a summary report to the appropriate levels of management of the corrective action taken. This report will include a trend analysis of nonconformities, including Murdocks and those of Approved Vendors."
Contrary to the above requirements, a summary report including a trend analysis was never prepared.
