UNION ASSOCIATES requests the NRC to provide all documents prepared or utilized by, in the possession of, or routed through the NRC containing any and all information related to the following.

1. All meetings and/or conversations between the NRC and LILCO and/or S&W regarding Mr. Dick's allegations.

Please include records or any telephone conversations as well as "person to person" conversations.

2. All participants in those meetings and/or conversations.
3. Any notes, minutes or other records generated by those meetings and/or conversations by the NRC, LILCO, S&W and/or othe parties
4. All documents prepared by LIL'CO or S&W for use during those conversations or meetings.
5. All documents prepared by the NRC for use at the meeting (s) or during conversations with LILCO and/or S&W.
6. All documents prepared as a result of inspections performed by the NRC as follow-up to Mr. Dick's allegations.
7. All documents showing the conclusions reached by the NRC regarding Mr. Dick's allegations.
