.'4 SPECIAL REPORT CATAWBA NUCLEAR STATION, UNIT 2 DIESEL GENERATOR 2B VALID FAILURE ON JANUARY 15, 1988 DUE TO DIESEL ENGINE GOVERNOR BEING OUT OF ADJUSTMENT A Valid Failure of Diesel Generator (D/G) 2B occurred on January 15, 1988, while Unit 2 was in Mode 6, Refueling. This was the third Valid Failure within the last 100 Valid starts of D/G 2B and the first in the last 20 Valid Starts.
Consequently, the current testing interval is 31 days, which is in accordance with Technical Specifications Surveillance 4.8.1.1.2.
There have been 3 Valid Failures within the last 100 Valid Starts on Unit 2 D/Gs.
After performing a full load rejection t'est per PT/2/4350/15B, Diesel Generator 2B Periodic Test, D/G 2B was shutdown after it could not be paralleled. Work requests 6174 PRF was initiated to investigate oscillating DC volts, DC Amp:/,
Frequency, and engine fuel rack position.
The cause of the oscillations was found to be the sencitivity of the D/G governor. Work Request 6174 PRF found that adjustaent of the D/G governor was necessary, and the adjustment was subsequently performed. Two Invalid failures occurred during governor adjustment. While troubleshooting the oscillation problem, 3 Invalid Failures occurred which are identified in another Special Report dated February 29, 1988. The periodic test was later satisfactorily completed.
D/G 2B had been previously removed from service after the Unit entered the current refueling outage. D/G 2A was operable during the period in which this Valid Failure occurred on D/G 2B.
