,o APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No.
50-336 Millstone Nuclear Power Station, Unit 2 License No. DPR-65 Waterford, Connecticut On October 27, 1987, during NRC inspection, an apparent violation of NRC require-ments was identified.
In accordance with the policy and procedures for NRC en-forcement actions, 10 CFR 2 Appendix C, that violation is listed below.
10 CFR 50 Appendix B Criterion XVI requires that measures be established to assure that conditions adverse to quality, such as failures and defective equipment, are promptly identified and corrected. Millstone 2 FSAR Section 12.8 states that the quality assurance program applied to operations phase activities involving safety-related systems and components commits to compli-ance with 10 CFR 50 Appendix B. Millstone 2 FSAR Section 9.9.15 states that:
the vital switchgear ventilation system functions to maintain a suitable en-vironment for safety-related electrical equipment during normal operation, loss of outside power, and post incident conditions; each redundant switchgear group shall have an independent subsystem having 100*; of the total required heat removal capability; and that the vital d-c siitchgear rooms are cooled by a chilled water system using service water as the ultimate heat sink.
Contrary to the above, measures were not established to promptly correct a condition adverse to quality in the vital d-c switchgear rooms.
Specifically, both redundant vital ventilation subsystem chilled water coolers (X169A and X1698) which use service water to cool the vital d c switchgear rooms under post-incident and loss of outside power conditions were identified as in-opera'le in 1983 and remained so until November 6, 1987, a period of about o
four years.
Inasmuch as alternate cooling means were provided by facility procedures and determined to be acceptable by licensee safety evaluation, this is a Severity Level IV Violation (Supplement 1.D).
In accordance with 10 CFR 20.201, the Northeast Nuclear Energy Company is hereby required to submit to this office, within thirty days of the date of the letter which transmitted this Notice, a written reply including: (1) corrective steps taken and results achieved; (2) additional corrective steps that will be taken; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
The response directed by this Notice is not subject to clearance by the Office of Management and Budget under the Paperwork Reduction Act of 1980, PL 96-511.
