APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company Docket:
50-382 Waterford Steam Electric Station, Unit 3 Operating License:
NPF 38 During an NRC inspection conducted December 16, 1987, through January 31, 1988, a violation of NRC requirements was identified.
The violation involved failure to follow procedures.
In accordance with the "General Statement of Policy and P?ocedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:
Criterion V of Appendix B to 10 CFR Part 50 and the approved quality assurance plan for Waterford 3 require that activities affecting quality be prescribed by documented instructions, procedures, or drawings.
Two examples of failure to adhere to procedures are delineated below:
1.
Procedure HE-4-809, Revision 4, "Low Voltage (600 volts and less)
Power and Control Cable / Conduction Termination and Splices," requires the component cooling water dry tower fan motor terminal splices to be constructed as shown in Detail 1 on Sheet 39 of Drawing LOV-1564-B-288, "Cable and Conduit List Installation Detail."
Contrary to the above, on December 30, 1987, the motor terminal splices on component cooling water dry cooling tower fan 4B were not constructed in accordance with Detail 1 on Sheet 39 of Drawing LOV-1564-B-288.
2.
Procedure MM-6-011, Revision 2, "General Torquing and Detensioning,"
requires Attachment 10.1, "Torquing Documentation Worksheet," to be 4
reviewed by quality control prior to commencing the torquing on safety-related equipment.
Contrary to the above, on January 6, 1988, the "A/B" essential service chilled water pump coupling half spacer and cover fasteners were torqued per Attachment 10.1 of Procedure MM-6-011 without a quality control review.
Items 1 and 2 constitute a Severity Level IV violation.
