99900105/78-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on June 5-9, 1978, it appears that certain of your activities were not conducted in full accord with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 and NCA 4134.5 of Section III of the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished i
in accordance with these instructions, procedures, or drawings."
Fisher Controls Company Administrative Practice Procedure 07-14, issued May 15,1977, Step 8, states in part, " Customer Service...
Initiates Rough Stock-Purchase Parts Inspection Report. Includes serial number... and other required information on the form including instructions...."
Contrary to the above requirements, it was observed that the Rough Stock / Purchase Parts inspection report for P0364421001 identifie l
the correct drawing number (55A1310), but not the latest revisic number (C) to the drawing used in performing the receiving inspection of part No. 55A1310 x 02AP.
