'O Walworth Company Aloyco Plant Docket No. 99900087/78-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 7-9, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 and paragraph NCA-4134.5 of Section III to the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings...."
WC-A procedure AWP-24, Revision 5, paragraph 1.6.3 states, " Temperature shall be determined by the use of contact pyrometer."
Contrary to these requirements, the welding interpass temperature was verified by the use of 3500F "Tempelstik" on a four inch (4") N-226SP Gate Valve Body, Purchase Order 30200, Sales Order 7193544.
The quality assurance engineer stopped the welding operation and instructed the welding supervisor that the temperature must be verified by the use of the contact pyrometer.
