ENCLOSURE 1 Arkansas Power and Light Company Arkansas Nuclear One, Unit No. 1 Docket No. 50-313 NOTICE'0F VIOLATION Based on the results of an NRC inspection conducted September 25 through October 22, 1978, it appears that certain of your activities were not' conducted in full compliance with conditions of your Operating License-No. 0PR-51 as-indicated below.
Technical Specification 6.8.1 requires.that written procedures be established, implemented and maintained.
Procedure 1005.04, Control.and Use of Bypasses and Jumpers, has'been established and in paragraph 6.1.6 requires that an' individual initial the jumper and bypass log sheets to verify.the-removal of jumpers or bypasses. -
Contrary to the above, Bypass Tag Numbers 77-121-1 and 77-121-2 which are logged on sheet #320 as having been removed by "R.B."
on March 15, 1978 do not appear to have been verified on removal as required by Procedure 1005.04.
This is an infraction.
