-QUALITY ASSURANCE REQUIREMENTS This program describes the quality assurance requirements for transport of sealed sources authorized by material license #13-16347-01 and owned by Columet Testing Services, Inc. (CTS). The sealed sources are used for Industrial rodlography.
103.0 QUALITY ASSURANCE ORGANIZATION The Radiation Safety Officer (RSO) is responsible to the President for implementing the Transportation Quality Assurance Program. The RSO has the responsibility to identify quality problems, provide solutions to quality problems, verify implementation of solutions and stop work when necessary.
The RSO is a rodlographer with at least one year experience. He has been trained in the requirements of this program. He is completely famillor with all the equipment used and functions corried out in the CTS radiography program. He has a thorough knowledge of management po!!cles and adminis-trative and operating procedures. He has a full understanding of rodlotion protection and control methods. His specific responsibilities are:
1.
Maintaining control of procurement and disposal of licensed material.
2.
Procuring and maintaining radiation survey Instruments.
3.
Establishing and maintaining storage facilities.
4.
Maintaining exposure devices, radiography facilities, and associated equipment.
5.
Performing source replacement and source tagging cpcrations.
6.
Conducting quarterly inventories and maintaining utilization logs.
7.
Implementing the Quality Assurance Program.
Responsibility for maintaining records as they pertain to the above functions con be designated to a responsible individual, but the ultimate responsibility for the records remains with the Radiation Safety Officer.

105.1 This program applies to transportation of the following radioactive materials packages:

For transportino Gammo Industries Model Century S exposure devices and Gamma Industries Model C-10 source changes (these devices may contain up to 100 curies Iridium-192)
USA /9036/B For tiansportIng Source Production & Equipment Company Model C-1 source changer (this device may contain up to 100 curies Iridium-192)
USA /9127/B For transporting up tc,100 curies Cobalt-60.
l This is a Gamma Industries Model Gammatron 100A exposure device.

105.2 Personnel who perform activities described in this program are l

105.3 The status and adequacy of the Transportation Quality Assurance Program is reviewed on on annual basis by trained personnel who do not have responsibilities in the orcos being oudited.

Inspection is performed in accordance with a written checklist. The checklist includes:
-Characteristics to be inspected
-Classification of personnel who are assigned to perform inspection
-Acceptance criteria
-Space for documenting results.
-instructions for handling nonconforming items.
121.2 Personnel performing inspections are trained to do so. Training is documented.
123.0 TEST CONTROL Not applicable to CTS transport activities.
125.0 CONTROL OF MEASURING AND TEST EQUIPMENT Not applicable to CTS transport activities.
127.0 HANDLING STORAGF. AND SHIPPING CONTROL Handling, storing and shipping is performed in accordance with written procedures. Cleaning and preservation are not applicable to CTS transport activities.
129.0 INSPECTION TEST AND OPERATING STATUS 129.1 The status of transport packages which do not comply with acceptance criterio is indicated by offixing on " INOPERATIVE" tog to the package.
A nonconformance report is completed and submitted to the RSO.
The status of transport packages which are not to0ged is " OPERATIVE".
131.0 NONCORFORMING MATEPfALS PARTS OR COMPONENTS Defective transport packages are removed from service and tagged. A Nonconformance Report is initiated and submitted to the RSO. The RSO determines and documents disposition.

scrapped or returned to the vendor for disposition 133.2 Defective packages type USA /9127/B(U) are also exposure devices.
Disposition of defective transport packages of this type is determined by the RSO in accordance with procedures determined by the 4
manufacturer.
