I, NOTICE OF VIOLATION Consumers Power Company Docket No. 50-255 As a result of the inspection conducted during the period of December 3-20, 1985, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:
Technical Specification 6.8.1.b, requires the implementation of written procedures for refueling operations. Palisades procedures; (1) FHS0-2, Revision 6, Refueling Procedure, and (2) S0P-28, Revision 6, Fuel Handling System, require that all loose hand tools, while in use over the reactor or fuel pool, shall be captured.
Contrary to the above, the licensee failed to provide and use the necessary restraining mechanism for loose tools used over the reactor and fuel pool. This resulted in a flashlight falling into the fuel pool tilt pit area.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
