NRC INTEGRATED INSPECTION REPORT 50 327/97 04 AND 50 328/97 04
Dear Mr. Kingsley:
This refers to the inspection conducted on April 13 through May 24, 1997 at
the Sequoyah facility. The enclosed report presents the results of this
inspection.
During the inspection period, your conduct of activities at the Sequoyah
facility were generally characterized by satisfactory operations and
maintenance. Operations performed appropriately in most cases, however,
during plant heatup and power escalation evolutions, failures to follow
,
operating procedures resulted in exceeding the power increase rate limitation
and pressure and temperature guidance. Additionally examples of poor logging
and communication were identified. Engineering implemented several steam dump
system modifications that should improve steam dump operations however,
maintenance activities on the valves were weak.
Plant support response to an
inadvertent spill was considered to be good.
Based on the results of this inspection, the NRC has determined that
violations of NRC requirements occurred. The violations are cited in the
enclosed Notice of Violation (Notice) and the circumstances surrounding them
are described in detail in the subject inspection report. Multiple failures
to adhere to o)erational procedures and one surveillance requirement were
identified. T1e NRC is concerned with the failure to adhere to procedures.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. The NRC will
