Stanley Site Vice President Braidwood Station Commonwealth Edison Company RR #1, Box 84 Braceville, IL 60407 Dear Mr. Stanley. , SUBJECT: NRC INSPECTION REPORT 50-456/97007; 50-457/97007 NOTICE OF VIOLATION Dear Mr. Stanley: On May 20,199?. the NRC completed an inspection at your Braidwood 1 & 2 reactor facilities. The enclosed report presents the results of that inspection. During the six-week period covered by this inspection period, your conduct of activities was generally characterized by safety-conscious operations. Specifically, the c augmentation of control room staff for the outage improved the supervision and ~ performance of evolutions and demonstrated a strong safety focus on shutdown operations. The operating staff's control room performance was characterized by l excellent teamwork and communications. We are concerned, however, about several specific problems: First, the inspectors identified that the Unit 2 motor-driven feedwater pump - discharge isolation valve was taken out-of-service in the open position disabling the valve's isolation function as described in the Updated Final Safety Analysis Report. This action was taken without first performing a safety evaluation in accordance j with 10 CFR 50.59. { l ' Second, the inspectors observed two examples where foreign material exclusion ! - barriers were not installed and the 1B diesel generator was unattended. 1 Third, iiie inspectors identified two examples where system engineers acting as tes l - directors failed to take the appropriate actions specified in testing procedures. Fourth, the inspectors identified three examples where procedures implementing the - Fire Protection Program were not followed.
