uoyah facility. The enclosed report presents the results of this

.inspec ion,

During the inspection period, conduct of activities at the Sequoyah facility
F
was generally characterized by adequate operations, maintenance and
engineering. Operators performed appropriately in most cases inspected,
however performance problems resulted in an unmonitored reactor coolant system
j
-
level decrease. This issue is discussed in a separate inspection report.

' Maintenance performance was considered to be mixed with a noted increase in
,
n
human performance errors. . Engineering performance was considered weak in
~
support of equipment operation, Reactor Trip Breaker testing, and the
j
_
Environmental Qualif1 cation program.
!

Based on the results of this inspection,'the NRC has determined that several
i
violations of NRC requirements occurred. The violations are cited in the
-
F
enclosed Notice of Violation (Notice) and the circumstances surrounding them
are-described in detail in the subject inspection report. A violation was
identified for a failure to adequately complete surveillance testing for the
>
Reactor Trip Breaker P 4 function. The NRC is concerned because your review
'of- a previous event-involving an inoperable P 4 channel, the subject of
l
' escalated enforcement, was not broad enough to identify this. Two violations -
were. identified with inadequate operations, ' maintenance, and engineering
l
-response to a_ degraded turbine driven auxiliary feedwater (TDAFW) pump. sump
i
=that could have affected the operability of the TDAFW pump. A violation was-
i
also identified for: failure.to control maintenance activities which caused two
!

incidents where non emergency power was lost to an emergency shutdown board.
Two. violations were' identified for ' deficient design control measures and
}
'
- untimely corrective action associated with Environmental Qualification (EQ) of
,

. equipment. - A non cited. violation.was identified involving circuit breaker

inoperability. The synopsis of an Office of Investigations report involving
falsification of a' radioactive materials shipment record is included as an'

attachment to the inspection report. This issue was also idenitfied as a non-
cited violation.
Finally, your review and proposed corrective actions for a violation which
caused a loss of cooling to the spent fuel pool was not sufficiently
comprehensive to address issues such as procedural usage of instrumentation,
flow indicator labeling, unapproved operator aids, and longstanding broken
equipment.
It therefore did not meet the NRC Enforcement Policy criteria for
a non cited violation.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to determine whether further enforcement action is
necessary to ensure compliance with regulatory requirements.
