SUBJECT: NRC INSPECTION REPORT 50-382/97-08 AND NOTICE OF VIOLATION
Dear Mr. Dugger:
An NRC inspectio1 was conducted Apri! 6 through May 17,1997, at your Waterford
Steam Electric Stat:on, Unit 3 reactor facility. The enclosed report presents the scope and
results of that inspection.
During the 6-week period covered by this inspection period, your conduct of activities at
Waterford 3 was generally characterized by good performance of operations, maintenance,
engineering, and plant support. However, violations of NRC requirements were identified.
Violation A discusses four examples of inadequate implementation of design control
measures. The first example involved a design change that added potential loads to an
emergency diesel generator without adequate controls to ensure the loads were maintained
below the value assumed in analysis. The second example involved the failure to
appropriately account for instrument uncertainty for the containment spray riser level that
resulted in operation outside the Technical Specification requirements. The third example
involved an extra layer of filter material added to a core protection calculator cabinet
without evaluating the potential effect on cooling air flow. The fourth example involved
numerous errors discovered in the Engineering Groundrules Document, which reflected a
lack of rigor in maintaining this c'esign basis document. The first and third examples were
NRC-identified, which indicates additional efforts are needed to ensure your current
programs are sufficient to identify design control errors.
Violation B was NRC-identified and involved the inappropriate deletion of preventive
maintenance instructions for safety-related breakers.
The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the j
enclosed Notice when preparing your response. The NRC will use your response, in part, I[
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements,
in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response (if you choose to provide one) will be placed in the NRC
Public Document Room (PDR). To the extent possible, your response should not include
