Gentlemen:

Subject: Inspection No. 50-213/86-04

A routine safety inspection of your solid radioactive waste preparation,

packaging and shipping program was conducted by Mr. H. Bicehouse of this office

on March 10-14, 1986, of activities authorized by NRC License No. OPR-61. Our

findings were discussed with Mr. R. Graves and others of your staff at the

conclusion of the inspection on March 14, 1986.

The areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter. Within these areas, the

inspection consisted of selective examination of procedures and representative

records, interviews with personnel and observations by the inspector.

The inspection indicated that your program failed to account for the presence

of Iron-55 in at least 18 radioactive waste shipments resulting in.three

apparent. violations of NRC requirements. In addition, your procedure failed to

provide acceptance criteria for verification of the identity and activity of

the radionuclides in radioactive waste shipments resulting in another apparent

violation of. NRC requirements. Details are provided in Sections 4 2, 6.1, and

.

6.4 of the enclosed Inspection Report.

We have reviewed the actions taken to correct the violations brought to your

attention in the enclosures to. our letters, dated August 6,1984, and June 29,

1985. The corrective actions stated in your letters, dated September 5, 1984,

and July 26, 1985, appear to be adequate to prevent recurrence. However, we

are concerned that your quality assurance program apparently failed to identify

the omission of Iron-55 in the waste shipment manifests and shipping papers.

The need for and nature of appropriate enforcement action relative to the

apparent violations will be the subject of separate correspondence at a later

time.
