NRC INTEGRATED INSPECTION REPORT 50-354/97-03
Dear Mr. Eliason:
On May 31,1997, the NRC cornpleted an inspection at your Hope Creek reactor facility.
The enclosed report presents the results of that inspection.
During this periad, several visiting inspectors conducted routine inspections of your site.
These inspections included reviews of actions taken to address loose or missing fasteners
for electrical covers on Seismic Category I motor control center and substation transformer
cabinets, routine evaluations of your radioactive liquid and gaseous effluent control and
environmental monitoring programs, and an oversight review of your recently completed
Station Service Water Operational Performance Inspection. The results of the latter
inspection will be detailed in a subsequent report. Overall, the results of these inspections
generally indicate that your radiological programmatic controls and quality oversight
programs are effective. Routine resident inspector observations and assessments also
noted generally good performance in the control and monitoring of safety-related activities.
We are concerned, however, about a violation of NRC requirements that was identified
during this inspection period. Specifically, we noted instances of failures by maintenance
personnel to adhere to written procedural guidance in the completion of safety-related
v.ork activities. Once such instance led to an undetected degradation of the high pressure
coolant injection system for a period of 11 days, a condition that was discovered only
through a failed quarterly inservice test of the system. Other examples were also
observed, including both NRC- and self-identified instances involving maintenance
procedure quality and compliance. Further, two recent Licensee Event Reports (97-04 and
97-06) document procedural errors by maintenance techniciar's. We are interested in your
response to this concern in light of this apparent negative tre,nd in performance, particularly
since corrective actions for similar NRC-identified issues in aarly 1996 (both during and
after the Restart Assessment Team inspection (see NRC !nspection Reports 50-354/96-80
and 96-03)) do not appear to have been fully effective.
Additionally, we noted that staffing levels in the Hope Creek engineering department have
declined over the past several months. While no direct observable performance issue has
been identified as a result of this decline, we remain concerned that your existing staff will
be significantly challenged to reduce the large backlog of engineering work, prepare for the
upcoming refueling outage, and conduct routine system monitoring in support of safe and
reliable operation over the next several months.
