APPENDIX A NOTICE OF VIOLATION i
Gulf States Utilities Docket:
50-458/86-05 River Bend Station License:
NPF-47 During an NRC inspection conducted on January 27-31, 1986, a violation of NRC requirements was identified.
The violation involved failure to maintain laboratory instrument calibration records associated with performing Technical Specification surveillance requirements.
In accordance with the "C9neral Statement of Policy and Procedure for NRC Enforcement Actions,' 10 tiR Part 2, Appendix C (1985), the violation is listed below:
l Failure to Maintain Calibration Records Technical @cification 6.10.2.d requires that " records of surveillance activities, inspections, and calibrations required by these Te'chnical Specifications" be retained for at least 5 years.
This is furthe~r amplified in River Bend Chemistry Section Procedure (CSP) - 0121, " Chemistry Laboratory Instrument Surveillance," and River Bend Chemistry Operating Procedure (COP) - 0425, " Determination of H2 and 02 Gas Concentration - Gas Chromatography Method," which establish frequency, methodology, and documentation requirements for calibration / standardization of the gas chromatograph.
Paragraph 8.3 in procedure COP-0425,-states, " Record the peak height, volume of standard injected, and attenuation setting, on the appropriate chemistry 4cg."
Contrary to the above, the NRC inspector determined on January 29, 1986, that the gas chromatograph ha'd,been used to perform 57 hydrogen analyses of the offgas hydrogen _recombiner effluent per~ Technical Specification Table 3.3 i.11-1, Action 123, without the required instrument calibration data being adequately documented on the appropriate cheraistry log and retained.
This is a Severity Level V violation (Supplement I)(458/8605-01).
Pursuant to'the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submi.t to this office, within 30 days of the date of letter i
transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be

given to extending the response time.
