This letter transmits to you the results of a special evaluation of the operational history for the D. C. Cook Nuclear Plant for the period January 1983 to September 1985.
Region III has initiated augmented assessments of licensees for the purpose of early identificatic' of weak performance to preclude the occurrence of significant events.
To accomplish this objective, a special task force was fomed to identify problems that may exist at the D. C.. Cook facility. The results of this evaluation report (attached) were discussed with you and members of your staff on November 13, 1985 at the Region III office in Glen Ellyn, Illinois.
The report concluded that two major problem areas exist that need ininediate attention - personnel errors and missed surveillances. There are numerous examples found in both areas which appear indicative of poor management control.
Furthermore, other problems were identified such as:
excessive ESF actuations involving radiation monitors; excessive numbers of cordition reports; lack of control over work activities; failure to evaluate trending data; unauthorized procedure changes; and failure to hcnor coninitments.
Consequently, you are requested to evaluate your existing management control systems and develop both short and long terin action plans to improve -
performance. Your action plans should specifically address: the projected Category 3 functional areas mentioned above; personnel errors; trending; condition reports; ESF actuations; and control of work activities.
