'N Please find enclosed Amendment No. 51 to your NRC material license.
You should review this amendment carefully and be sure that you understand all conditions.
l If you have any questions, you may contact the reviewer who signed your license amendment at 817/860-8100.
In order to expedite the section of your May 10, 1989, amendment request changing your designated Radiation Safety Officer (RS0), it was necessary to amend your license with the following stipulations and resttictions:
1.
Your license has been amended without inclusion of the October 1, 1988, FAMC Reg 40-604 document.
In order to facilitate review of this document by NRC and to allow inclusion of this regulation in your radiation safety program, it will be necessary for you to identify the specific changes made in the August 1,1986, FAMC Reg 40-604 document.
Indicate by proof reading bars in the text margin, as well as, identify by page number and paragraph location, the indicated changes in these two documents. This mode of editing will allow for a complete and thorough review of requested changes.
Your October 1, 1988, FAMC Reg 40-604 documents are being returned to you for editing.

Upon receipt of the edited information and completion of technical review by NRC staff, we will amend you license to include the FAMC Reg 40-604 document. As this is a continuation of the May 10, 1989, amendment request, you should, when responding to this letter, reference Control No.
462588.
3.
The title Assistant Radiation Safety Officer (ARS0) is not cited anywhere in NRC rules and regulations.
It is the intent of 10 CFR that one person, the RSO, be responsible for the radiation safety program at any given institution.
Delegating RSO authority to an ARSO would not allow the RSO to retain complete responsibility for radiation safety.
Creating the position of ARSO would dilute the intent of the regulations. We are, based on the above, not acknowledging the title ARSO, and accordingly, are not evaluating Mr. 2:ed's credentials at this time.

Operate in accordance with NRC regulations 10 CFR Part 19, " Notices, Instructions and Reports to Workers; Inspections," 10 CFR Part 20,
" Standards for Protection Against Radiation," and other applicable regulations.
2.
Possess radioactive material only in the quantity and form indicated in your license.
3.
Use radioactive material only for the purposes indicated in your l
license.
4.
Notify NRC in writing of any change in mailing address (no fee required if the location of radioactive material remains the same).
5.
Request and obtain' appropriate amendments if you plan to change ownership of your sole proprietorship or partnership, change the corporate status of your. company, change locations of radioactive material, or make any other changes in your facility or program which are contrary to your license conditions or representations made in your license application and any supplemental correspondence with NRC. A license fee may be charged for the amendments if you are not in a fee-exempt category.

. Submit a complete renewal application with proper fee, or termination request at least 30 days before the expiration date on your license.
You will receive a reminder notice approximately 90 days before the expiration date.
Possession of radioactive material after your license expires is a violation of NRC regulations.

in accordance with 10 CFR Part 170.
Failure to conduct your program in accordance with NRC regulations, license conditions, and representations made in your' license application and supplemental correspondence with NRC will result in 4
enforcement action against you. This could include issuance of a notice of violation; imposition of a civil penalty; or an order suspending, modifying, or revoking your license as specified in the General Policy and Procedures for NRC Enforcement Action, 10 CFR Part 2, Appendix C.
Since serious consequences to employees and the public can result from failure to comply with NRC requirements, prompt and vigorous enforcement action will be taken when dealing with licensees who do not achieve the necessary meticulous attention to detail and the high s andard of compliance which the NRC expects of its licensees.
