Final 10CFR50.55(e) Report: Service Water Spill (CDR 85-00-15)

Dear Sir:

On September 18, 1985, we reported a potential 10CFR50.55(e) deficiency

[ Reference (b)], regarding a service water spill in the Primary Auxiliary Building (PAB). This deficiency has been determined reportable under the provisions of 10CFR50.55(e).

Description of Deficiency A piping elbow in the A Service Water Train downstream of Valve V-15 was removed for remedial work on the elbow lining. Subsequently, an estimated 30,000 gallons of seawater from the downstream end of the piping was spilled on the top floor of the PAB, eventually flowing down on components on lower floors.

An investigation of the event determined that it was caused by the decision of the tagging authority to leave Valve V-20 open and to accept the open-ended pipe. The open end was at the system highpoint so that even with the other train of service water in operation and not isolated from the open

.end, all flow was down to the discharge structure. When the tunnel heat treatment controls were tested, closing the service water return to the discharge structure, the return valve to the intake structure also failed to open, eliminating any discharge flow at the system low point. Water exited the system overflow line and the open pipe.

United States Nuclear Regulatory Commission Attention: Mr. Richard W. Starostecki Fage 2 The magnitude of the clean-up effort caused the item to be reported under the provision of 10CFR50.55(e).

Safety Implications The wetted items included safety-related stainless and carbon steel piping systems and electrical and control equipment.

There were no personal injuries.

Corrective Action A thorough check of all PAB equipment was made to ensure the identification of all potentially affected equipment. Exterior surfaces of piping systems were rinsed with demineralized water and swipe tested. Results indicated that the rinsing was adequate to remove any residual seawater contamination and that no potential damage could have resulted.

With minor exceptions, wetting of electrical equipment was confined to external surfaces, which were subsequently dried. Those areas with internal moisture were cleaned and dried.

Appropriate disciplinary action has been taken with the test engineer and the details of the event have been discussed with the Shif t Test Engineers.
