PROPOSED SITE VISIT - NOVEMBER 13, 1985

. This is to confirm our plans to have staff members from the NRC Office of Analysis and Evaluation of Operational Data (AE0D) visit the River Bend Station on November 13, 1985. The purpose of the visit is to collect and discuss infor-mation on an operating event which occurred on September 23, 1985 involving a partial draindown of -the River Bend Station reactor vessel.

The detailed agenda for this visit is delineated in the enclosure to this letter. The proposed NRC attendees at this visit are Messrs. S. Rubin and P. Lam of AE00. The visit would last from 7:30 a.m. to 3:30 p.m. on i November 13, 1985.

Please inform NRC Project Manager, Stephen Stern, if there is a need to change the schedule for the proposed visit, whether the staff could use photographic equipment on the visit as well as any clarification or further discussion of 4

Proposed Topics for Discussion for River Bend Plant Visit __

Purpose:

To collect and discuss information on the operating event on 9/23/85 involving the draindown of the reactor vessel.

Specific areas of interest for review / discussion / collection of information:

1. Tour the River Bend reactor building to visually examine the installation arrangements and elevation of the LPCI and RHR shutdown cooling pipings and isolation valves.
2. Review and collect P& ids on the RHR system and its subsystems (LPCI and shutdown cooling) and drawings involving the ele-

. vation and configuration of the refueling cavity and spent fuel .

3. Discuss and obtain detailed step-by-step sequence of events relating to the vessel draindown event on 9/23/85. Obtain procedures used for valve manipulations prior to and during this event. -

Discuss the independent verification practices related to the verification of procedural steps for valve manipulations prior to and during the event.

4. Obtain information on operability requirements / practices far containment isolation function when in cold shutdown /refurling modes. Discuss operability requirements for emergency cr e cooling systems when in these modes. Also discuss automatic actuation systems operability during these modes.
5. Discuss draindown rate (RCS level drop rate) and time available for operator actions as well as automatic actuation levels relative to the event.
6. Discuss the plant's implemented responses to IE Information Notice 84-81 and GE SIL 388. *
7. Review and discuss the plant's implemented corrective actions to prevent recurrence of the referenced operating event.
8. Visit the control room to walk through the (simulated) valve manipulations and indications available to the operator to diagnose the draindown and to aid in preventing recurrences.
9. Disuss the interlocks' to prevent valve line-ups which can cause draindowns. Discuss the absence of interlocks which allowed this event to occur.
