Exceeded daily maximum limit of 0.20 mg/L.

Description of Incident:
During chlorination of "B" cooling tower, the total residual chlorine was measured at 0.24 mg/L. Certified operator reviewed data on October 30 and became aware of exceedance and corrective action.

Apparent Cause:

60 gallons from the preceding amount of 50 gallons per chlorination.

The chlorination was immediately terminated. Will reduce the amount of chlorine used to 50 gallons or less during chlorination.

The following day 40 gallons of chlorine per cooling tower were used with the maximum amount of TRC in the discharge being 0.10 mg/L.

Description of Incident:
Routine sample of the turbine sump oil separator tank on October 21, 1985 (0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br />) was reported on October 25, 1985 to be 26.0 mg/L.

Apparent Cause:
During the previous week the floors in the turbine building had been scrubbed down in preparation for painting. Excess oil and dirt on the floor was susequently rinsed to the turbine floor drains and eventually the oil water separator.
5.
If not inanediately corrected, the anticipated time the condition is expected to continue:
NA 6.
Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
None required. Second sample taken on October 25, 1985 to ensure return to normal operation had 11.7 ag/L.

Additional Comments:
Believed this to be an isolated event. Previous weekly sampling had 8.3 and 8.0 mg/L. Oil depth in the turbine sump oil separator tank was 1/16 inch on October 24, 1985. No oil removal equipment had been bypassed. No oil was observed on mixing base discharge or on the lake before, during or af ter the incident.

Description cf Incident:
The oil / water separator was removed from service on 9/20/85 for scheduled cleaning and maintenance. An extra sample for Outfall 00F collected at 1745 hours0.0202 days <br />0.485 hours <br />0.00289 weeks <br />6.639725e-4 months <br /> on 9/21/85 during this cleaning measured 34.2 as/L oil and grease.
The sample results were received on 9/26/85. The turbine sump oil separator tank (2,000 gallon capacity) prior to Outfall 00F remained in service during this cleanout period.

Apparent Cause The oil / water separator was due for maintenance and cleaning after eight months of service. The oil separator tank was left in service to remove oil 1
and grease from the effluent; however, it apparently failed to remove sufficient oil to maintain compliance with effluent limits.
5.
If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence This incident is an isolated occurrence and i not expected to reoccur.
Emptied the oil separator tank and cleaned the oil ski mers and replaced the oil absorbent. The oil / water separator was returned to service at 1305 hours0.0151 days <br />0.363 hours <br />0.00216 weeks <br />4.965525e-4 months <br /> on 9/21/85. The oil / water separator tank remained in service. A sample for Outfall 00F collected on 9/23/85 measured 8.5 mg/L oil and grease.

Additional Coments:
The outfall observation of 9/21/85 showed no oil visible in the discharge.

The receiving water shall contain no unnatural... oil film... in quantities which are or may become injurious to any designated use as a result of this discharge.

Description of Incident:
011 from an as yet unknown source surfaced in the makeup basins overflowing to the mixing basin and into Lake Michigan. About ten gallons of turbine lube oil were involved. Sufficient oil in B pump basin to gather into globules; oil in Lake Michigan sufficient to yield rainbow colors about 50 feet out into the lake (about one-half gallon per acre).

Sorbent booms were placed in the makeup basins and a sweep was placed in Lake Michigan. Testing of main lube oil coolers will be done to locate oil source.

Oil released from the cooling towers was observed to cause a sheen in Lake Michigan in the immediate area of the discharge only. No oil was visible along the shoreline at any time.
3.
Description of Incident:
Shortly after starting each cooling tower pump lubricating oil was observed in the makeup mixing basins. Estimated at less than two gallons of oil.
4.
Apparent Cause:
An accumulation of oil in the cooling towers was washed through the cooling system upon start-up of the cooling tower pumps.
5.
If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
Placed and replace absorbent pillows in each makeup basin and in the mixing basin to collect oil and prevent its release. Sampled various systems to determine source of oil. None observed. A&B Industrial placed double oil booms in the makeup basin to eliminate discharges. These booms were subsequently replaced. Examined each cooling tower fan motor and gearbox for oil leaks and generate Maintenance Order to repair such leaks.

Description:
Discharge to Lake Michigan Outfall Code No: 800095 (Outfall 001)
Parameter: pH Parameter Code No: 00400 1.
Date of Incident: 5/23/85 2.
Identification of specific effluent limitation or monitoring requirement affected:
pH limit of 6.0 to 9.0 3.
Description of Incident:
From 12:00 M to 1:00 AM on 5/23/85, approximately 3,000 gallons of acidic demineralizer regenerants were accidentally released from the Neutralizer Tank T-921 which apparently caused the final discharge to the lake to fall below the lower pH limit of 6.0.
The discharge pH may have been as low as 5.5.
A grab sample collected at 1:00 AM measured 6.05 pH in the discharge, confirming the end of the apparent exceedence.
4.
Apparent Cause:
The release from the neutralizer tank occurred when the tank was filled above its internal overflow standpipe. The standpipe overflows directly to the mixing basin which discharges to Lake Michigan. The tank contents had not been neutralized at the time of the overflow. The overflow occurred as a result of problems incurred during the resin bed regeneration which required extra volumes of water.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
The tank overfill was stopped before 1:00 AM on 5/23/85 when the regeneration was completed. Neutralization of the Tank T-921 contents was completed by 5/24/85 at 5:00 AM. The tank was released normally at that time. Maintenance was performed on the equipment which caused the malfunction during the regeneration. Maintenance was also performed on the neutralizer tank level indicator.

Additional Comments:
The exact pH value in the discharge could not be determined because maintenance on the primary pH sonitor left a calibration error preventing measurement below 6.0 pH (see attached sheet). The " backup" pH monitor was operable but its chart recorder was undergoing maintenance at the time.

Description of Incident:
Routine weekly grab samples for oil and grease collected from the internal monitoring point Outf all 000 (Tank T-41), on 2/4/85, 2/10/85 and 2/11/85, measured 58.2, 24.8 and 25.6 mg/L, respectively.

Apparent Cause:
Apparent oil carry-over from the new API oil waste separator to Tank T-41 5.
If not immediately corrected, the anticipated time the condition is expected to continue:
NA 6.
Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence Performance of the new API oily waste treatment system continues to be evaluated and fine tuned to plant operations. As a result of the high readings, the interior of the separator was cleaned and accumulated oil removed from Tank T-41.
The separator's oil skimming weir was adjusted to manufacturer's specifications. System performance monitoring samples were also collected and analyzed.

The final discharge to Lake Michigan contained no visible oil on the receiv-ing waters.

Break in acid line on pump discharge to cooling towers.
Nature of Loss Unclude Complete Description of Demogel The majority of the acid was contained in the trench and pumped into the tower. The trench was analyzed at 13% acid; therefore, about 1 gallon escaped the trench and ran down the road (about 5-10 gallons of total liquid). This liquid was diluted with water then neutralized with line. with lime to neutralize the acid. This line will be placed in DOT 55 gallon drums for disposal. Plant Management recognizes the seriousness of this problem and has launched a concerted effort to provide a suitable solution to this problem.

An squipment aalfunction caused an unexpected shutdown and malfunction of "A" cooling tower water flow controls.
A surge of water in the cooling tower basin caused an overflow of the basins. This flood of water collected at the south end of the turbine building, filling a 55 gal barrel with water. The partially buried 55 gal drum 4
is used as a catch basin during fuel unloading at the T-10 fill connection. The residual fuel oil (Cont on Page 2)

2220 Operations starts up P-39A cooling tower pump.
Thursday Aurust 22,-1985 0825 During daily observation of discharge basin, Chemistry Technician observes small amounts of oil in make up basins and mixing basin and reports same to Shif t Supervisor. The oil observed in the sixing basin was only in the quiet corners where the surface was stable.
There was no oil observed in Lake Michigan.
Operations Department placed several absorbant pillows in each make up basin and in the mixing basin to collect the oil and prevent its release.
Notifications to MIDNR were not considered necessary as no oil was observed in the lake.
Saturday August 24, 1985 II.45 Operations starts P-39B cooling tower pump.
1512 Security Officer notifies Chemistry Department that there was oil in the aske up basins, mixing basin and in Lake Michigan. Chemistry Department notified Shift Supervisor and
. attempted to notify Environmental Coordinator.
1546 Shift Supervisor notified Duty and Call Superintendent.
1620 Duty and Call Superintendent notified Michigan " Pollution Emergency Assessment System (PEAS)" and USEPA National Response Center.
Operations Department replaced the absorbant pillows and increased the number in each of the three basins.
1900 No oil' visible in lake at this time, event considered terminated.

'0900 Oil observed leaking through seams between pillows in aske up basin into mixing basin and on/out into Lake Michigan. The til in the sake basin was enough to just give colors to the sheen while in the mixing basin and in ke Michigan a sheen was observed in places but with g color.

This loss was discovered at 4:30 PM on September 19, 1985 and reported to the Plainwell District MDNR at 9:00 AM on September 25, 1985.
