SUBJECT: NRC INSPECTION REPORT 50-382/97-04. AND NOTICE OF VIOLATION

Dear Mr. Dugger:

An NRC inspection was conducted February 23 through April 5,1997, at your Waterford

Steam Electric Station, Unit 3 reactor facility. The enclosed report presents the scope and

results of that inspection.

,

During the 6-week period covered by this inspection period, your conduct of activities at

Waterford 3 was generally characterized by good performance of operations, maintenance,

engineering, and plant support. However, violations of NRC requirements were identified.

,

Violation A identified numerous examples of the failure to comply with the Technical

Specification requirements for the use of overtime. The violation reflects a weakness in

management oversight and ownership for the control of overtime, which could affect the

quality of safety-related activities.

'

Violation B involved the failure of your staff to recognize that Shield Building Ventilation

Train B failed its Technical Specification surveillance for maximum air flow and your

,

surveillance review process allowed the surveillance failure to go undetected for a period of

time greater than the Technical Specification allowed outage time for that equipment.

Further, your failure to recognize that Train 8 did not comply with the surveillance

i

requirements allowed the other train to be taken out of service for maintenance without

recognition that both trains would be rendered inoperable.

Violation C involved two examples of inadequate 10 CFR 50.59 evaluations. Personnel

involved in the preparation and review of these evaluations demonstrated inadequate

t

knowledge of Technical Specification requirements for containment isolation valves. As a

result of these inadequate evaluations, an instance was identified of failure to comply with

the Technical Specification requirement for direct position indication for containment

isolation valves, which caused the Technical Specification allowed outage time to be

exceeded without the required actions being taken.
