SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-213/96-13
Dear Mr. Feigenbaum:
On January 5,1997, the NRC completed an inspection at Connecticut Yankee Atomic
Power Company. The enclosed report presents the results of that inspection.
During the seven-week period covered by this inspection period, the conduct of activities
at the Haddam Neck Station facility was generally characterized by the actions to secure
from the Cycle 19 refueling outage and to prepare plant systems for preservation as plans
to decommission the plant were finalized. In accordance with your letter per 10 CFR
50.82 to the Nuclear Regulatory Commission (NRC) dated Decernber 5,1996, the
authorization under License DPR-61 to operate the reactor was relinquished. Maintenance
and operating activities were significantly reduced, except as needed to assure safe

storage of the fuelin the spent fusi pool, and to continue to meet the requirements of the
operating license. Station staff initiated plans to reduce and reorganize the plant staff for
the transition from an operating to a decommissionirg organization.
Several performance issues were noted during the inspection. During the conduct of a
surveillance test on the emergency diesels, the failure by an operator to follow a test
'
procedure resulted in the operation of the "B" emergency diesel with the jacking tool
installed. The fLilure to follow procedures was a violation of NRC requirements, as
described in the enclosed Notice of Violation. Although there was no apparent damage to

the "B" emergency diesel and the safety significance of the operator error was minimal,
this matter remains a concern to the NRC due to the history of operator and personnel
performance issues at Haddam Neck. Other performance deficiencies were noted in the
actions to install a blank flange on the fuel transfer tube, and to evaluate vendor

information related to the seismic qualification of relays. In your response to the enclosed
Notice, please describe in particular your plans to enhance performance standards, along
I
with the actions that will be taken to improve procedural adherence by all station workers.
Other areas inspected included your program for radiation protection, including
management organization, effectiveness of program implementation and a review of the
Northeast Utilities Nuclear Services Company (NUSCO) Dosimetry Laboratory. Although
your programs in these areas were determined to be generally effective, a general decline
in the implementation of your radiation protection program was observed.
