CONFIRMATORY ACTION LETTER CAL-RIII-85-14 October 23, 1985 Docket No. 50-409 Dairyland Power Cooperative ATTN: Mr. J. W. Taylor General Manager 2615 East Avenue - South La Crosse, WI 54601 Gentlemen:

This letter confirms the telephone conversation between Mr. Dwane Boyd of this office and Mr. John Parkyn of your staff on October 23, 1985, related to an apparent improper response of the reactor protection system which resulted in an alert and manual rod insertion during a startup on October 23, 1985.

It is our understanding that an alert was declared when an operator noted that an annunciator panel had activated during startup, indicating high neutron flux but that automatic protection system actuation did not occur as he believed it should have. He immediately terminated the startup by manually inserting all withdrawn control rods. With regard to the matters discussed, we understand that you will:

1. Conduct a thorough review to determine if a trip signal actually was received by the Reactor Protection System (RPS).
2. If such a signal was received, determine why the control rods failed to automatically insert.
3. If such a trip signal was not received, determine why the annunciator was
activated.
4. Determine what caused the signal that nuclear instrumentation observed and why nuclear instrument channel number 5 was not affected.
5. Maintain all affected equipment related to the Reactor Protection System in such a manner that it can easily be kept or placed in the "as found" condition. Therefore, take no action which would destroy or cause to be lost, (other than necessary to protect the health and safety of the public) any evidence which would be needed to investigate or reconstruct this event.
6. Review operator actions taken imediately following his recognition of the failure to trip and determine if these actions were in accordance -

7. A formal report of your findings and conclusions will be submitted to the Region III office within 30 days.
8. We further understand that startup will not occur until authorization to restart is obtained from the Regional Administrator or his designee.

Please let us know ininediately if your understanding differs from that set out above.
