Inspection 50-289/85-21
During the period from August 2 to September 16, 1985, members of the NRC
TitI-1 Restart Staff conducted routine and special safety inspections of
shutdown activities at your facility. The staff documented the results of
this inspection in the enclosed inspection report. At the conclusion of the

inspection Mr. R. Conte, of my staff, summarized the !nspection findings for
you and other members of your staff. The findings wera generally favorable
with no observations of conditions adverse to nuclear safety.
The enclosed report describes the staff's review and evaluation of two
significant events that occurred during the inspection period. These events
are the partial loss of non-nuclear instrumentation that occurred on August
21,1985,and the fire in the reactor's rod control system that occurred on
-
September 7, 1985. Although no adverse safety impact resulted,'we consider
them to be operationally significant. We detennined that operating personnel
were responsive to these events in that they properly used emergency
procedures and their plant knowledge to assess the unique circumstances and
take appropriate corrective actions. The training program was a factor in
contributing to the appropriate response of operators to these events.
In
response to these events and other less significant events, licensee represen-
tatives properly implemented applicable administrative controls and provided
.
an appropriate level of review to determine adequate corrective actions. Most
notable of these was the troubleshooting effort following the fire in the rod
control system.
Of particular interest to us in reviewing the first of these events is that
certain non-nuclear instrumentation can be lost as a result of a single
electrical failure. Although this instrumentation is backed up by other
instrumentation, the latter is not in all instances safety grade. We noted
that your emergency procedures call for operators to rely directly or
indirectly on some of these instruments to direct certain operator actions.
Altnough this instrumentation does not perform automatic safety functions, we

meet regulatory requirements as set forth in the enclosed Hotice of Violation
(AppendixA). The violation resulted from a security guard failing to conduct
a proper search of an escorteo visitor, contrary to your security plan and
procedures. Uc are satisfied with your insnediate and followup corrective
measures to prevent recurrence. Accordingly, you need not respond to this
letter.

program that have been detemined to be exempt from public disclosure in
accordance with 10 CFR 73.21, Safegu:rds Infomatfor.. The sections so
