APPENDIX A NOTICE OF VIOLATION Cor nell University Docket No. 50-157 Ward Laboratory of Nuclear Engineering License No. R-80 As a result of the inspection conducted on April 22-24, 1985, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

10 CFR 55, Appendix A requires that research reactors shall have requalifica-tion programs for licensed senior reactor operators. The licensee's Requali-fication Program for. Licensed Operators states that an annual written requali-fication test will be given.

Contrary to the above, on April 2;2, 1985, written requalification tests had been given to one senior operator on February 28, 1982 and April 22, 1985, and to another senior operator on February 26, 1982 and December 20, 1983.

These frequencies exceed the required annual test frequency.

This is a Severity Level V Violation (Supplement I.D).

Pursuant to the provisions of 10 CFR 2.201, Cornell University is hereby required to submit to this office within thirty days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including: (1) the cor-rective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full com-pliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
