SUMMARY
Scope:
This routine, unannounced inspection involved 36 inspector-hours on site
in the areas of concrete expansion anchor bolts associated with IEB 79-02
inspection, safety-related pipe support and restraint systems, and licensee
actions on previous enforcement matters.
Results: ~ 0ne violation was identified - Failure to follow procedures for
concrete expansion anchor bolt inspection, paragraph 5.0.

Licensee Action on Previous Enforcement Matters

Violation 327, 328/85-49-01, Inspection and installation problems
for seismically designed pipe supports.
The inspector reviewed the draft
response regarding the corrective steps taken by the licensee.
It was noted
that a walkdown inspection was performed by the licensee inspection team
which consisted of one engineer, two craftsmen, and one Quality Control (QC)
inspector._ The inspection covered only in the auxiliary building.
The
supports inspected were not documented in the sample program. The inspector
advised the licensee that the supports inspected should have been documented
and readily available for NRC's review..
In addition, other areas which
contain safety-related pipe supports should also be included in-the sample
program.
Pending completion of the necessary corrective actions by the
licensee, this item remains open.

Unresolved Items
Unresolved items are matters about which more information is required to
determine whether they are acceptable or may involve violations or devia-
tions.
One new. unresolved item identified during this inspection is
discussed in paragraph 7.0.

5.
Concrete Expansion Anchor Bolts Associated With IE Bulletin 79-02 Inspection
The inspector performed a field inspection of concrete expansion anchor
bolts associated with the pipe s~ pport installations in the auxiliary
u
building to determine whether they had been properly installed and inspected
in accordance with licensee commitments and NRC requirements. The inspector
noted that at least one concrete expansion anchor bolt was not properly
installed and inspected for each of the pipe supports and one steel column
baseplate identified below:

The above nine pipe supports and one baseplate for the pump support platform
were partially inspected.
It was noted that the anchor bolts associated
with the supports and the baseplate were identifi' d to have visual gaps
e
between the boltheads and the baseplates.
In some cases, the gaps were
found to be even greater than i inch in length.
TVA Sequoyah Nuclear Plant 79-02 Anchor Bolt Inspection procedure requires
f
the correct installation of self-drilling concrete anchor bolts in safety
systems per General Construction Specification G-32 and TVA's inspection and
testing program.
Paragraph 3.2.5 of Specification G-32 states that the
installers are to be instructed to tighten the bolts between 1/8 and 1/4
turn after the bolthead comes into contact with the attachment or baseplate.
The licensee's QC inspectors failed to detect the anchor bolt deficiencies
for the aforementioned pipe supports and the baseplate for the pump support
platform during the inspection.
This is a violation of 10 CFR 50,
Appendix B, Criterion V, and is identified as Violation 327, 328/86-16-01,
Failure to Follow Procedures for Concrete Expansion Anchor Bolt Inspection.
Within the areas inspected, one violation was identified.

Pipe Support and Restroint Systems in Conjunction With The Previous
Violations
During the inspection the inspector found discrepancies for the following
supports-
-
Uniti 1, Auxiliary Feedwater System Support No.1AFDH-350, one jam
nut was missing.
Unit 1, Chilled Water System Support No. 1031 HPABHVAC-080-04,
-
one nut was missing from the load bearing bolt connection.
-
Unit 1, Fire Protection System Support No.1026 HPABFS-075-016,
the ears of the 6" standard-pipe strap were not properly
tightened to the supporting plate for which approximately
1" gap was observed.
Discrepancies identified from the above supports are aoditional examples to
the previous violation item. 327, 328/85-49-01.
The licensee's corrective
actions with respect to the violation should also include the aforementioned
supports.

Within the areas inspected, no violations or deviations were. identified.
7.
Evaluation of HVAC Duct Support
During the inspection,
the NRC Region I NDE Team identified potential
problems associated with missing welds, unpainted, uncleaned, and possibly
uninspected support attachments for the HVAC supports 1030 HDABHVAC-0820,
0822 and 7965, located above elevation 734 in the auxiliary building.
At
the time of this inspection, the licensee was not able to provide adequate

information with regard to the above concerns. Pending more information to
be furnished by the licensee for further review, this matter is identified
as Unresolved Item 327/86-16-02,
