09/01/85 At the beginning of the month the Ouane Arnoi d Energy Center was un der I

normal power operation with 427 MWe (gross) being pro du ce d.

l 09/04/85 At 1050 hours0.0122 days 0.292 hours 0.00174 weeks 3.99525e-4 months the Reactor Core Isolation Cooling (RCIC) System failed a i -

mo n t h l y operability test when the RCIC turbine trippe d on its first manual start.

The plant entered a 7 day LCO.

Repairs were m a ck to the mechanical overspeed trip mechanism and the 7 day LCO was terminated at 2205 hours0.0255 days 0.613 hours 0.00365 weeks 8.390025e-4 months on 9/09/85 (LER 85-035) l 09/06/85 At 1400 hours0.0162 days 0.389 hours 0.00231 weeks 5.327e-4 months both airlock doors between the Reactor. an d Turbine Bu i l ding were in a dver t en t l y opened simultaneously, resulting in -a violation of secon dary containment.

(LER 85-038) 09/09/85 At 1425 hours0.0165 days 0.396 hours 0.00236 weeks 5.422125e-4 months an LCO 'was entered when the diesel fire pump failed to start

The LCO was cancelled at 1610 hours0.0186 days 0.447 hours 0.00266 weeks 6.12605e-4 months on 9/13/85 At 1430 hours0.0166 days 0.397 hours 0.00236 weeks 5.44115e-4 months it was discovered that during a previous electrical inspection, an open penetration approximately 3 inches in di a me t er was foun d in the fire barrier between the High Pressure Coolant injection (HPCI) end j

Reactor Core Isolation Cooling (RCIC) rooms.

A continuous fire watch was j

established until the penetration could be s ea l e d.

No wiring or other i

equipment was in the penetration, an d fire de t e c t i o n and Suppression equipment was available on both si des of the penetration.

09/10/85 At 1203 hours0.0139 days 0.334 hours 0.00199 weeks 4.577415e-4 months a 14 day LCO w as~ en tere d when a Stan dby Diesel Generator Room fire zone alarm became I n o p e r a b l e.' The alarm was place d back in service after repairs were done on the Instrument cable, thus en ding the LCO at 1454 hours0.0168 days 0.404 hours 0.0024 weeks 5.53247e-4 months on 9/12/85 09/11/85 At-0837 hours the

'A' train of the Stan dby Gas Treatment System was remove d from service' for planned filter cleaning, placing the plant in a7 day LCO.

The filters were cleaned, placing the system back in service and en ding the LCO at 1526 hours0.0177 days 0.424 hours 0.00252 weeks 5.80643e-4 months on 9/12/85 09/_12/85 At 1330 hours0.0154 days 0.369 hours 0.0022 weeks 5.06065e-4 months a momentary secon dary violation occurred when both doors on the Stan dby Gas Treatment Room were opened at the same time.

09/14/85 At 0828_ hours the plant entered a 30 day LCO when the

'A' Main Steam Line isolation Valve - Leakage Control System (MSIV-LCS) was dec l a r e d inoperable due to a logic relay failure.

The relay prov i des - an annunciator function only on' loss of control power. 'The system was dec lare d operab le when the Operations Department determined control power could be veri f ied re dun dant ly by operators on the MSIV-LCS panel.

The 30 day LCO was then canceled at j

'B' Emergency Diesel Generator was declared Inoperable upon falling a monthly operability test.

The diesel fuel oil transfer pump'had failed to supply the required amount of fuel to the day tank.

A faulty limit switch was recalibrated and tested satisfactorily, thus ending the LCO at 1538 hours0.0178 days 0.427 hours 0.00254 weeks 5.85209e-4 months .

09/18/85 At 1018 hours0.0118 days 0.283 hours 0.00168 weeks 3.87349e-4 months during the RCIC operability test, a RCIC-to-vessel injection valve failed to open on command placing the plant in a 7 day LCO.

The torque switch in the motor operator was replaced and the test was successfully completed at 1951 hours0.0226 days 0.542 hours 0.00323 weeks 7.423555e-4 months on 9/19/85, thus ending the LCO.

(LER 85-039 pending) 09/24/85 At 0510 hours0.0059 days 0.142 hours 8.43254e-4 weeks 1.94055e-4 months the

'D' RHRSW pump was removed from service entering a 30 day LCO for planned maintenance. The pump discharge check valve was overhauled and placed back in service at i200 on 9/27, thus ending the LCO.

09/26/85 At 0833 hours0.00964 days 0.231 hours 0.00138 weeks 3.169565e-4 months the

'D' MSIV-LCS was declared inoperable for repairs on a flow transmitting device.

A 30 day LCO commenced, which was in day 5 at the end of the month.

At 1630 hours0.0189 days 0.453 hours 0.0027 weeks 6.20215e-4 months the Emergency Notification System was declared inoperable.

AT&T was notified and the system was placed back In service at 2030 hours0.0235 days 0.564 hours 0.00336 weeks 7.72415e-4 months .

09/30/85 At 0610 hours0.00706 days 0.169 hours 0.00101 weeks 2.32105e-4 months the

'B' RHRSW pump was removed from service for planned maintenance on the pump discharge check valve.

A 30 day LCO was entered.

.The plant was in normal power operation with 571 MWa (gross) going out to the grid.

We remained in a 30. day LCO for repairs on the

'D' MSIV-LCS and a 30 day LCO for maintenance on the

'B' RHRSW p ump discharge check valve.
