SUPPLEMENT TO CONFIRMATORY ACTION LETTER Rill-97-002 l I

Dear Mr. Mueller:

J On September 19,1996, Unit 2 was shut down for a refuelmg outage. On February 21,

- 1997, Unit 1 was shut down due to an inoperable containment spray pump. During the Unit 1 reactor shutdown, a reactivity management event occurred. Due to concerns with performance deficiencies that may have occurred during this event involving supervisory oversight, crew communications, procedure quality and adherence, plant staff safety i focus, control room decorum, and operator understanding of expectations regarding reactivity control, the NRC issued a confirmatory action letter (CAL) Rill-97-002 which formalized your commitments to take specific actions in response to this event. In addition, the NRC dispatched an Augmented Inspection Team (AIT) to determine the facts associated with this event. The AIT identified human performance deficiencise involving

both the operating crew and licensee management and concluded that the event resulted from a breakdown in command and control by operations supervision, inadequate communications within the operations department, the failure to pre-plan the shutdown

! evolution, and licensed operator knowledge and training deficiencies. You decided not to restart either unit until you had corrected human performance problems in operations, material condition issues affecting startup, and deficiencies in engineering support to operations that have been identified by the NRC and your staff. This supplemental CAL is being issued to formalize your commitment to take specific actions to address these i deficiencies.

l (1)  ! Develop a recovery plan for Zico Station to address corrective actions for the -

various performance deficiencies referenced above and transmit that plan to the l NRC for formal public docketing, in developing this plan, you will also establish quantitative and qualitative criteria for assessing and monitoring the effectiveness ,

of your corrective actions for each of the areas identified in the recovery plan.  ;

The extent to which any action will not be completed prior to restar: will be identified and assessed to determine if restart is acceptable with action pending.

(2) You will keep the NRC updated on the projected startup date for the lead unit in  :

order to facilitate advance planning of an NRC special inspection to be conducted .l prior to startup. This NRC inspection will evaluate the operational readiness of Zion l Station and your staff.

(3) You will meet with the NRC staff on a periodic basis to discuss the results of the recovery plan implementation and the extent to which the evaluation criteria for  !

corrective action effectiveness have been met. These meetings will be open to >

public observation.

(4) You w;41 meet with the NRC once you have completed your operations department personnel assessment process and subsequent remediation training to discuss both the results of this selection and training initiative and the process and indicators you intend on using to evaluate the abi!!ty of operating personnel to startup and operate the units at TJon Station. This meeting will be open to public observation.

(5) You will meet with the NRC once you have concluded that one of the units at Zion t Station is ready for restart to discuss tim basis for your conclusion. During this meeting, you will discuss the results of planned independent assessments of recovery program implementation effectiveness and Zion Station operational readiness by Zion Stations's Site Quality Verification organization and Safety Review Board, and Commonwealth Edison's Nuclear Operations Division management team. This meeting will also be open to public observation.

Nothing in this confirmatory action letter is intended to preclude you from taking actions which you deem necessary to place the plant in a safe condition or maintain it in a safe condition in the event of changing conditions.

Pursuant to Section 182 of the Atomic Energy Act,42 U.S.C. 2232, you are required to:
