Interim 10CFR50.55(e): Possible Malfunction of Figure 3050 Diaphram Valves Supplied by Dresser Industries (CDR 85-00-18)

Dear Sir:

On October 23, 1985, we reported a potential 10CFR50.55(e) deficiency, Reference (b), regarding Figure 3050 Diaphram Valves supplied by Dresser Industries. We have completed our evaluation and have determined that the deficiency is reportable.

Description of Deficiency Dresser Industries reported a possible malfunction in their Figure 3050 Diaphram Valves. The valve disc cap is capable of cocking in the disc guide and wedging, thereby not allowing the disc to move. In this instance, the upper stem may be raised with the disc remaining in a closed or part partially closed position.

Safety Implication The safety related systems these valves are installed in are the Combustible Gas Control and Nitrogen Gas Systems.

In the Combustible Gas Control System, this valve is used in the con-tainment hydrogen analyzer lines. There are two independent and redundant hydrogen analyzer systems located outside the containment to continuously monitor the containment hydrogen concentration following a design basis loss of coolant accident. Failure of the valve (s) to open would render the hydrogen analyzers inoperative and prevent containment hydrogen sampling capabililty.

The Nitrogen Gas System valves are normally closed and are not required to operate during or _ following an accident.

Corrective Action We intend to implement the modifications to the disc guide as recommended by Dresser and will inform Region I when these modifications are complete.
