Gentlemen:

.This. refers to the routine safety inspection conducted by Mr. L. J. Hueter

of this office on January 13-17, 1986, of activities at Fermi 2 authorized

by NRC Operatipg License No. NPF-33 and to the discussion of our findings

with F. E. Agosti and other members of your staff at the conclusion of the

~. inspection and by telephone on February 3, and 7, 1986.

The' enclosed copy of our inspection report identifies areas examined during.

the inspection.

Within these areas, the inspection consisted of a selective

examination of procedures and representative records,. observations, and

interviews with personnel.

,

No violations of NRC requirements were identified during the course of this

,

inspection.

However, we are concerned that personnel errors were apparently

-

a root cause of all six occurrences involving the area of gaseous and liquid

effluents reviewed during this inspection.

Specific corrective measures had

been instituted in each case.

In addition, we understand that two programs,

the recently implemented Reactor Operations Improvement Program and

'

Radiological ~ Improvement Plan being developed, are intended to address a

generic approach for . resolution of the recurring problem of personnel errors.

We will monitor the effectiveness of these programs during future inspections.

!

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter and the enclosed inspection report will be placed in the NRC

Public Document Room.

We will gladly discuss any questions you have concerning this inspection.
