Box 10429 Southport, NC 28461 , SUBJECT: NRC INTEGRATED INSPECTION REPORT N05. 50-325/97-02, 50 324/97 02 NOTICE OF VIOLATION Dear Mr. Campbell: On March 1,1997, the NRC completed an inspection at your Brunswick reactor facility. The enclosed report presents the results of that inspction. During the six weeks covered by this inspection period our inspectors found that your staff generally took a safety conscious approach to the activities conducted at the Brunswick plant. We are concerned, however, about six violations of NRC requirements that were identified. The first violation was for failure to follow an o)erating procedure for restoring an open valve to a locked closed position. T11s violation was of concern because independent verification failed to detect this error. The second violation was a maintenance rule violation for failure to adequately do a historical search of system surveillance testing. This violation was of concern because the reactor protection system circuit breakers were not properly classified and were being incorrectly monitored. The third violation was for failure to have adequate design control measures for design verification of configuration changes. This violation was of concern because design verification is required to be performed to insure that the design basis is properly translated into design output. The fourth violation was for failure to implement an inspection program for safety related miscellaneous structural steel. This violation was of concern because the inspection requirements to ensure proper installation had been deleted. The fifth violation was for failure to take prompt corrective action once a discrepancy was identified for an Emergency Core Cooling System stroke time values. This violation was of concern because when this discrepancy was first identified no action was taken to correct plant p ocedures to indicate the correct stroke time values. The

' sixth violation was for failure to have chemicals identified as required by . quality assurance requirements. This violation was a concern because of the I prcgrammatic problems identified.
