Enclosed is a copy of the revised NRC Form 398. This revised fc,rm requires that all infonnation be completed in total with each new, renewal, upgrade, multi-unit, and reapplication license submittal. Applicants requesting a renewal and reapplication will also need to indicate the additional training received since the last application under Item 15. Please note that the
-training section will no longer be cumulative. Other changes are detailed in. is a set of instructions for the c<mpletion of all items on the NRC Form 398.
Please ensure that all applications for licenses received after April 1,1986,-
are submitted on this revised form. On that date, the NRC Operator Licensing
' Tracking System will be modified to reflect these changes, and submittals using the previous form will not be accepted.
Additional copies of the form can be obtained from Vivian Miller, U. S. Nuclear Regulatory Commission, Document Management Branch, Mail Stop W-U.S Washington, DC 20555.
A meeting will be held with all Region III licensees in late March or early April to discuss this and other operator licensing 4tters. You will be notified of the details of this meeting at a later date.
