Enclosed is a copy of the revised NRC Form 398. This revised form requires that all information be' completed in total with each new, renewal, upgrade, multi-unit, and reapplication license submittal. Applicants requesting a renewal and reapplication will also need to indicate the additional training received since the last application under Item 15. Please note that the training section will no longer be cumulative. Other changes are detailed in. is a set of instructions for the completion of all items on the NRC Form 398.
Please ensure that all applications for licenses received after April 1,1986, are submitted on this revised form. On that date, the NRC Operator Licensing Tracking System will be modified to reflect these changes, and submittals using t
the previous form will not be accepted.

A meeting will be held with all Region III licensees in late March or early April to discuss this and other operator licensing matters.
