Limitorque Motor Operators (CDR 84-00-09)
Dear Sir:
In Refere'nce'(b), we submitted an interim report on a number of deficiencies regarding Limitorque motor operators. The final status of the deficiencies (Items 1, 4 and 5) which were previously determined to be reportable per 10CFR50.55(e) are as follows:
Item 1 Loose Contact Screws on Contact Bridge of Torque Switch
)
The fiber shims used under the contact bridges of torque switches on safety-related SMB-000 and SMB-00 operators have been replaced with metal shims to prevent the loosening of the contact screws.
Item 4 Improper Lug Connections Lug connections, crimps and wiring have been inspected on all safety-related operators.
Identified deficiencies requiring repair or replacement have been completed for all but eighteen operators. Although the wock on the eighteen operators is not yet complete, the subject was discussed with Mr. A. Cerne, NRC Senior Resident Inspector, and was determined that it is acceptable to close out this item because the remaining work is being controlled with a work request.

Mr. Richard W. Starostecki Page 2 Item 5 Sheared Tabs on Phenclic Cam of Torque Switch Torque switches on safety-related SMB-00 and SMB-000 operators have been inspected and changed where necessary to ensure that only the type with metal locking keys are utilized.
With the above corrective action having been verified completed, this is considered our final report on this item.
