Interim 10CFR50.55(e): Manual Handwheel Operator on Dampers Supplied by Pacific Air Products Company (CDR 85-00-16)
Dear Sir:
On October 17, 1985, we reported a potential 10CFR50.55(e) deficiency, Reference (b), regarding manual handwheel operators on safety-related dampers supplied by Pacific Air Products Company. We have completed our evaluation and have determined that the deficiency is reportable.
t Description of Deficiency Several safety-related dampers at Seabrook Station were furnished by l'
Pacific Air Products Company with Contromatics actuators. These actuators were provided with manual operators consisting of a handwheel mounted on a threaded rod which screws into an extended threaded travel stop. Although the primary mode of actuation is pneumatic, manual positioning of the damper with the handwheel operator will override pneumatic actuation.
During the testing phase of the pertinent HVAC systems, these handwheel operators were used occasionally to cycle their respective dampers in the..
absence of pressurized air.
In several cases, the threads in the extended travel;stop fouled causing the operator and damper to seize.

Mr. Richard W. Starostecki Page 2 Safety Implications This handwheel operator design deficiency could have affected plant safety in only one case where system operation depends on the operators i
to perform a safety-related function. This case involves the isolation dampers on the remote makeup air intake lines for the control room. Both dampers automatically isolate their respective intakes upon a high radi-ation signal from either intake. The damper isolating the intake which did not generate the high radiation signal must then be manually opened
-with the handwheel operator. If the operator and damper were to seize in a closed or partially closed position, control room habitability may be affected.
Corrective Action Modifications' recommended by Pacific Air Products to preclude operator /
damper seizure during normal damper cycling are currently being implemented.
We will inform Region I when these modifications are complete.
