On 7/14/94, Unit 2 utahty lic,= ,I Jt==cear Operstors co====<=rl the Reactor Protective System penodse logic sastra test.

While testing the fourth logic matra (AC matrix), Trip Carasit Breaker (TCB) Frve failed to open. TCB 5 had opened three timo previously on demand from other matru tests. Following several ====* to open TCB-5, it was dedared out of semce.

i The rator was thus down to repair or replace TCB-5.

j The cause of the failure of TCB 5 to trip has been identified as obstruction of motion on the trip latch lever due to a broken piece of inestating matenal. A sinall piece of broken phanale insulatang anssenal ,.,,.--- A 1/4 inch square in aime was found lodgea Seeween the trip insch lever and the access slot in the bem for the trip latch. The phanal c fragment was found to have enginated from the cutoff swach used in the breaker antipump d.-7, which is physically located duecaly above the trip istch =arkism. Upon further ==p ch it was determined that a screw used in the assembly of the caeof swiech had become loose. Th> screw holds the switch to the m~~'ag block and if loose will allow the two halves of the awnch to separate when the beaker is operated. This most likely led to Switch :-P and breakage of the phenohe block.

T e weir Te At 0230, while testing the fourth logic matrix (AC matrix) of the B= rear Protocuve System JC),(RPS)(E Trip Circuit Breaker (TCB) Five (See Figure One)(EIIC.AA) failed to open. TCB-5 had just opened three time

The utility le.a-d Senior Reactor Operator attempted to open TCB 5 locally with the local pushbutton, the emerge j

r" and by pulhas the breaker fuses. All attempts were unsuccesful. The Reactor Trip pa=kke ran for channel MA TCBs 1 and 5 was depressed without success. At 0330 TCB-5 was declared out of servux. TCBs 1,2 and 6 were opened 0418, which opened the series breaker and the parallel supply path to the Contml Element Drive M-ekaa:*- bus (EII the 'A' side. .

At 0740 a reactor shut down was co==>-r d in order to repair or replace TCB 5. The TCBs and the CEA Bus Motor Generator (MG) Set Output Breakers (EDC:ED) are General Electric (GE) Type AK 2 25. TCB-5 was replaced and the 1 sning Unit 2 TCBs and MG Set Output Brunkers were inspected ='-ba-ily. Unit 2 was then restored to 100% ste

, power operations.

s CAUSE OF THE EVENT i

I The cause of the failure of TCB.5 to trip has been identified as obstrucuon of motion on the trip latch lever. A small p broken phenolic insulating material approwimar.ly 1/4 inch square in size was found lodged between the trip latch lever the access slot in the base for the trip lates . When the breaker was called upon to trip, electrically or manually, the piece restricted the motion of the trip latch lever such that the breaker enachnaiam would not operate. Upon further inspection, it was determined that a screw used in the ausmbly of the cutoff swnch had become loose. This screw holds the swach to the mounting block and if loose will suow the two halves of the swach to separate when the bre

is opwated. This most likely led to swnch '"7 ~ and brunkage of the pbsenlic block.

Three violations were identified for (1) the failure to meet Technical Specification requirements to maintain the-Pressure Operated Relief Valves (PORVs) operable when at low pressure conditions; (2) the failure to adequately identify and perform post-maintenance' testing of the PORVs; and (3) the failure i to perform adequate inservice testing of the PORVs. We propose that the issue be classified as a Severity Level III problem consistent with Supplement I.C.2.a of the Enforcement Policy: A system designed to prevent or mitigate a serious safety event not being able to perform its intended function under certain L

conditions. Application of the civil penalty assessment process would have '

resulted in a fully mitigated civil penalty but due to the poor licensee performance in this case, discretion was applied to impose the base civil penalty.

This case is not exempt from the Office of Enforcement's timeiiness requirements.
