Gentlemen:

Subject: Resident Inspection 50-213/85-21 (10/16/85-12/02/85)

This transmits the findings of the routine NRC inspection of the Haddam Neck Plant

on October 16 - December 2, 1985. These findings have been discussed with Mr.

R. Graves of your staff.

This inspection cited, as a violation, multiple instances of failure to conform

to the requirements of your administrative control procedures. While these speci-

fic failures did not result in any actual adverse plant operating conditions, we

are concerned that they may be indicative of a lack of concern for procedure ad-

herence on the part of the facility staff. Therefore, in addition to your reply

to the individual violations, please include your evaluation of the effectiveness

of your quality assurance program in assuring conformance with its implementing

procedures and describe your plans for improving that conformance overall. Also,

in regard to Violation Citation 1.f, please address the failure to change surveil-

lance procedures as necessitated by design changes as that relates to the effec-

tiveness of your corrective actions on the reactor cavity seal failure and the

associated December 1984 NRC Order.

The second violation cited was for failure to adequately correct a problem with

overdue procedure reviews. This item is worth particular note because it is for

inadequate correction of an identified problem. -

We also wish to call Report Detail 6.4 to your attention. This item deals with

apparent interaction between steam flow measurement channels. Although the cor-

rective action described does address the problem symptom, we did not find that

the root cause and its implications have been identified.
