EVALUATION OF ROOT CAUSE FINDING REPORTS FOR PLANS NO. 5, 6 AND 7 (SFRCS) AND 15A and ISB SOURCE RANGE NUCLEAR INSTRUMENTATION We have completed our review of Toledo Edison Company root cause findings and corrective actions reports for plans 5, 6 and 7 relating to the Steam and Feedwater Rupture Control System (SFRCS) and ISA and ISB relating to the Source Range Nuclear Instrumentation. The evaluations relating to our reviews of these plans are included as Enclosures 1 and 2 to this letter.

Based on our reviews of these plans we have concluded that there is reasonable assurance that Toledo Edison Company has identified the fundamental causes of the malfunctions experienced with these systems, and that Toledo Edison has taken appropriate corrective actions to prevent recurrence. Note that with regard to additional testing to be conducted on the effects of hydraulic and electronic noise on the steam generator level instruments, the staff will review the testing results and provide a final evaluation of your root cause determination. We have, however, concluded that the results of your analysis to date are valid and, therefore, this final determination is not required prior to a decision permitting restart of the unit.
