Dear Mr. Kingsley:

This refers to the inspection conducted on January 19 through March 1,1997 at
i
the Sequoyah facility. The enclosed report presents the results of this
i
inspection.
During the inspection period, your conduct of activities at the Sequoyah
facility were generally characterized by good operations and maintenance.
,
l
Operators responded appropriately to a turbine runback event and to a decrease
in condenser vacuum event. Maintenance exhibited good performance in
,
!
addressing emergent issues during planned emergency diesel generator outages
!
and in improving the switchyard predictive maintenance program. However, the
l
,
NRC is concerned with continuing 3roblems associated with the heater drain
t
tank level control system and tur)ine runback circuitry which resulted in a
l
demand for a runback and an unexpected runback response.
Based on the results of this inspection, the NRC has determined that a
,
violation of NRC requirements occurred.
Several examples of an inadequate
,
alaric response procedure were identified. The violation is cited in the
!
enclosed Notice of Violation (Notice) and the circumstances surrounding it is
described in detail in the subject inspection report.
,
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. The NRC will
'
use your response, in part, to determine whether further enforcement action is
necessary to ensure compliance with regulatory requirements.
