Dr. Zack Pate, President Institute of Nuclear Power Operations 1100 Circle 75 Parkway, Suite 1500 Atlanta, Georgia 30339
Dear Dr. Pate:
SUBJECT:
CASE STUDY REPORT - LOSS OF SAFETY SYSTEM FUNCTION EVENTS The Office for Analysis and Evaluation of Operational Data has finalized its case study of events involving a total loss of safety system function (LSSF).
The study identified 133 LSSF events in the 1981 to June 1984 time period, 87 of which (65%) were the result of human factors contributions.
The final report addresses the peer review comments provided by NRR, IE, Region II, INPO, l
B&W Owners Group, Baltimore Gas and Electric Company (Calvert Cliffs), and Florida Power Corporation (Crystal River 3).
We have enclosed a copy of our final report for your information.
Although the study found no evidence of any previously unrecognized safety problems, it did find that over the time period considered in the study there is no clear evidence of improvement in preventing loss of safety system events.
Therefore, the report contains recommendations that are intended to increase awareness of the causes and contributing factors of this type of event.
The recommendations are currently being reviewed by the Nuclear Regulatory Commission staff.
A copy of the final case study report and this letter are being placed in the Public Document Room at 1717 H Street, N.W., Washington, DC 20555.
If you have any questions regarding this report, please feel free to contact Gene Trager of my staff.
Mr. Trager can be reached at (301) 492-4495.
