CASE STUDY REPORT - LOSS OF SAFETY SYSTEM FUNCTION EVENTS The Office for Analysis and Evaluation of Operational Data has finalized its case study of events involving a total loss of safety system function (LSSF).

The study identified 133 LSSF events in the 1981 to June 1984 time period, 87 of which (65%) were the result of human factors contributions. The final y report addresses the peer review comments provided by NRR, IE, Region II, INPO, B&W Owners Group, Baltimore Gas and Electric Company (Calvert Cliffs), and Florida Power Corporation (Crystal River 3). We have enclosed a copy of our final report for your information and appropriate action.

Although the study found no evidence of any previously unrecognized safety problems, it did find that ever the time period considered in the study there is no clear evidence of improvement in preventing loss of safety system events.

Therefore, it is recommended that IE feed back the results of the study to industry. In addition, it is recommended that NRR review training and qualifications development programs to ensure the programs adequately consider the types of personnel actually involved in LSSF events.
