l APPENDIX A NOTICE OF VIOLATION Vermont Yankee Nuclear _ Power Corporation Docket No. 50-271 Vermont Yankee Nuclear Power Station License No. DPR-28 Vernon, Vermont As a result of the-inspection conducted on October 21 - November 25, 1985, and in accordance with the revised NRC Enforcement Policy (10 CFR'2, Appendix C), published in the Federal Register on March 8, 1984 (49 FR 8583), the violation discussed below was identified.

A. Technical Specification (TS) 6.5.A requires that detailed written surveillance procedures be prepared, approved and followed. Procedure OP 4511 was written pursuant to the above and requires test personnel to expose the Refuel Floor Area Radiation Monitors (ARMS) to a radiation source of sufficient strength-to activate the channel trip'setpoint, and further, to note and record the trip settings during the quarterly calibration.

Contrary to the above, during quarterly calibrations on July 26 and October 28,1985, the East and West Refuel Floor ARMS were not exposed to a radiation field of sufficient intensity to activate the channel trip setpoints. Fur-ther, supervisory personnel who approved the completed tests failed to detect the discrepant test results and initiate corrective actions.

This is a Severity Level V Violation (Supplement I.E).

Pursuant to the provisions of 10 CFR 2.201, Vermont Yankee Nuclear Power Corpora-tion is hereby required to submit to this office within thirty days of the date of the letter which transmitted this notice a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved, (2) corrective steps which will be taken to. avoid further violations, and (3) the date-when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
