SUBJECT: NRC INSPECTION REPORT 50-482/97-04 AND NOTICE OF VIOLATION
Dear Mr. Maynard:
An NRC inspection was conducted January 11 through February.22,1997, at your Wolf
Creek Generating Station reactor facility. The enclosed report presents the scope and
results of that inspection.
During this inspection, five violations were identified in tne engineering, maintenance,
operations, and plant support areas. Violation A is of concern because it is an example
where your corrective action evaluation was not sufficiently broad to identify this issue
during your preparation of Licensee Event Report 50-482/96 10. Violations B, C, and D
are of concern because they demonstrate continued problems in controlling activities
-
associated with the turbine-driven auxiliary feedwater pump. While the plant staff
identified some of the examples of these violations, and others were self-revealing,
enforcement discretion is not considered appropriate for these violations because of the
number of significant problems that you have had with the turbine-driven auxiliary
feedwater pump during the past year. After reviewing your responses to these three
violations, we will consider whether a management meeting is needed to further discuss
the continuing problems with activities associated with the turbine-driver auxiliary

feedwater pump. Violation E is of concern because you inadvertently reduced your
emergency planning effectiveness, and your corrective actions were not prompt in that the
shif t supervisor was not informed of the error in the emergency action levels for 7 days.
These violations are cited in the enclosed Notice of Violation (Notice), and the
circumstances surrounding the violations are described in detail in the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response. The NRC will
use your response, in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
Two unresolved items were identified concerning your use of vendor technical manuals and
cold overpressure mitigation associated with the installation of your normal charging pump.

level of detail used in the review that resulted in your corisidering your vendor technical
manuals as design documents, in the second case, we will be reviewing the

circumstances that may have created the potential for inadequate cold overpressure
mitigation during a plant cooldown. These issues are discussed in Sections E1.1 and
'
E1.2 of the enclosed inspection report.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosures, and your response will be placed in the NRC Public Document Room (PDR).
To the extent possible, your response should not include any personal privacy, proprietary,
or safeguards information so that it can be placed in the PDR without redaction.
Should you have any questions concerning this inspection, we will be pleased to discuss
them with you.
