SUMMARY

- UNIT NO. 2 OCTOBER 1985 SALEM UNIT NO. 2 The Unit began the period operating at full power.

On 10/07/85 at 1001 hours0.0116 days <br />0.278 hours <br />0.00166 weeks <br />3.808805e-4 months <br />, the Unit tripped coincident with a safety injection as a result of a voltage spike on the 2C Vital Instrument Bus.

The voltage spike was caused by the use of an improperly wired 115V receptacle.

On 10/09/85 at 2106 hours0.0244 days <br />0.585 hours <br />0.00348 weeks <br />8.01333e-4 months <br />, with the completion of investigations and repcirs, the Unit was placed on line and was returned to 100% power.

On 10/30/85 at 1515 hours0.0175 days <br />0.421 hours <br />0.0025 weeks <br />5.764575e-4 months <br />, the weld upstream of valve 2SJ168 was discovered to be leaking.

The repair involved tagging out both flowpaths to the Boron Injection Tank and entering Action Statement 3.0.3.

In accordance with the Action Statement, a unit shutdown was commenced at 2001 hours0.0232 days <br />0.556 hours <br />0.00331 weeks <br />7.613805e-4 months <br />.

Sub sequently, the repairs were completed and the shutdown terminated on 10/31/85 at 0056 hours6.481481e-4 days <br />0.0156 hours <br />9.259259e-5 weeks <br />2.1308e-5 months <br />.

At 0917 hours0.0106 days <br />0.255 hours <br />0.00152 weeks <br />3.489185e-4 months <br />, Unit 2 was restored to full power where it remained for the rest of the period.
