In reference to your letter dated November 25, 1985 regarding our Quality i
Assurance Program Approval No. 0392. Since our origina'l approval this company has changed locations twice. During this. time we had been scheduled to merge with and be taken over by another testing' laboratory who his,since gone bankrupt.
A lot of our original paperwork was lost during this turmoil.
Would you please send un a copy of our past Quality Assurance Program.and, if necessary, an application so that we may take steps to re-instate our Quality Assurance Program Approval.
Currently our new Radioactive License No. 20,-19067-01 is still awaiting approval by Mr. John Glenn of,the U.S. Nuclear Regulu ;ory Comissior, Region I Office, King of Prussia, PA.
